How can you possibly tell?

Perhaps a head should roll further up the management pyramid but it would be incredibly unfair to punish someone for the deeds of others.

Besides, the codes of practice will all show that hand-washing is vital and should be done between every patient contact therefore their back is covered - they can't be held accountable if someone ignores the rules.

Unofortunately, it is a fact that when you're doing something hour after hour, day after day, year after year there is a tendency for corners to be cut. The non-handwashers will have done this before and got away with it.

Something like this brings the dangers back to the fore and, for a while, there may well be extra diligence with regard to handwashing but it will gradually slip back into the bad habit of non-washing until the next tragedy occurs.
